One of the best tools for conducting online surveys (including pre-made surveys you can use "as-is" or further customize to meet your own needs) is Profiler. After creating a free account, you can look at survey instruments created by others, modify one of them as desired, or create your own from scratch.

Another worthwhile online sample is one from Sun Associates. An example of a robust survey system created to meet the needs of a large group of educators is PILOT (Preparing to Integrate Learning Opportunities with Technology) from educators in Washington state.
