Web-based request forms like these can be created with free help-desk software like Web+Center 4.0 and RequestCommander. A large number of commercial help-desk solutions are available, but freeware/shareware options may meet your needs and save both money and development time in trying to create an entirely home-grown web form solution for your tech support office.
